Page cache needs to be cleared after enabling a Captcha for forms and anonymous users.

If you add a CAPTCHA intended for anonymous users to a form and use page caching, the CAPTCHA will not appear properly until the page cache has been refreshed. This will be fixed in the next version of MyCaptcha.
If you experience trouble with the current behaviour, execute the following query after enabling CAPTCHAs for anonymous users:
TRUNCATE cache_page;
